Farmworker Case Management Across Borders
An Effective Practice
Description
The Farmworker Case Management Across Borders (CAB) program provides primary and preventive health care to migrant and seasonal farm workers. The Putnam County Health Department and St. Vincent’s hospital of Jacksonville use a Mobile Health Unit and partnerships with local medical and dental centers to offer care free of charge. The program helps farm workers manage their chronic diseases, complete treatment for communicable diseases, and access preventative dental and vision care. Clinics offer night hours once a week, with expanded service during peak cabbage and potato season. Outreach workers provide transportation to and from the night clinic, specialty appointments, and health education services.
Goal / Mission
The goal of the Farmwork Case Management Across Borders program is to improve migrant and seasonal farm worker health.
Results / Accomplishments
As of 2002, CAB had provided 28,069 adult and 814 child health encounters. Farm workers had accessed 553 dental exams, 297 vision exams, 373 chronic disease consultations, 585 communicable disease treatments, and 1,702 family planning encounters. The Case Management Across Borders program was recognized in March 2000 as a Model Program for Primary Care in Underserved Populations by the Department of Health’s Quality Improvement Review Committee.
